Step by Step

This post responds to a suggestion made by a co-resident. Here step by step to start the process to obtain a medical license in NYC, as well as how to apply to the boards. Please feel free to write in the comments your questions about the process, I will try to help you as much as I can.

FEDERATION CREDENTIALS VERIFICATION SERVICE

Most likely you already have a profile on FCVS with all your basic information. You need 606 dollars to get the certification from FCVS.

  1. Designations> Medical Board> New York State board for Medicine
  2. Update personal information. Skip if it’s the same.
  3. Education and certification. Skip if it’s the same.
  4. Licensure and activities. Fill chronology of activities since graduation- You can’t have any gaps.
  5. Documents.
    1. Affidavit and certification of identification
      1. Affidavit- Print the form, you need a passport picture attached to the form. You can get it notarized downstairs by the entrance at the cashier’s office (2 dollars). If you don’t want the hassle, there is the option of having online notarization (25 dollars).
      1. Certification of identification- You can use your passport or birth certificate. This needs to be notarized too.
    1. Supporting documents
      1. Names: Use your passport
      1. Medical education: Upload your diploma and translation in English.
      1. Accredited program: Certification of completion of residency. – Not applicable if currently in residency.

MEDICAL LICENCE IN NEW YORK

You need 735 dollars.

  1. Professions > Medicine  > Physicians
  2. Application forms > Online form 1
  3. Pre-requisites > Apply for a new license> Begin application.
  4. Personal information > Fill out your personal information.
  5. Application
    1. I wish to become licensed on the basis of > acceptable examination scores.
    1. Using FCVS > Yes
    1. History> Fill here if you have another name on your diploma.
    1. Moral character> History of malpractice
    1. Medical education: Fill here your education starting high school. Fill clerkships if any (you can remove the form if you did not complete any clerkships)
    1. Fifth pathway licensure> No
    1. Specialty qualifications: Remove.
    1. FSMV: I have completed the examinations:
      1. USMLE steps 1, 2 and 3. You can find the date of passing step 3 on your FCVS profile
    1. Graduation date of medical school
    1. Post-medical school activities: Chronological activities from graduation from medical school until the start of residency.  Once you complete residency you will submit another form.
    1. Previous licenses > No
    1. Child abuse identification and reporting coursework requirement
      1. I graduated from an NYS registered program and completed the child abuse identification training as part of my studies. You upload the certificate on https://eservices.nysed.gov/professions/addl-doc/

They have a list of the approved training providers. (I checked at least 20 sites: about 10-30 dollars. Here the 2 cheapest options: 1. https://fastceforless.com/child-abuse-new-york/ – 2. https://www.netce.com/specials.php)

PEDIATRICS BOARD TEST REGISTRATION

You need 2,288 dollars.

  1. Create a username on American Board of Pediatrics.
  2. Apply for the exam:  Initial certification in general pediatrics.
  3. Application form: Fill in your personal information > Medical degree: You will need an ECFMG number> Licensure: You don’t need a license to apply.
  4. Pay with a credit card or debit card.
  5. If you want to get the reimbursement from CIR you have to log into their portal

    1. Submit a claim > Professional Education Benefit > Claim form > Board exam fees > Attach receipt.
    1. To get the receipt of the test. Log into the profile of the American Board of Pediatrics. My ABP portfolio> Documents archive> Receipts
